When people in services for people with learning disabilities talk about transition they usually mean what happens when a young person with learning disabilities leaves school.
That as young people grow up, getting the support they and their family need should be smooth and easy.
Young people should have the same chances in their lives as everyone else.
Objective : As young people with learning disabilities move into adulthood, to ensure continuity of care and support for the young person and their family; and to provide equality of opportunity in order to enable as many disabled young people as possible to participate in education, training and employment.
Growing up is an exciting time, with new chances and choices.
But it is stressful and difficult for many young people with disabilities.
This is because the organisations in their lives do not talk to each other well enough, and they do not involve the young person and their family enough in planning for their future.
Also if they don't have good health plans, they can't use the adult services they want to.
